metropolitan area. We currently provide services in eight counties:
Anoka, Dakota, Hennepin, Isanti, Ramsey, Sherburne, Washington, and

We provide trained direct care professionals who offer one-to one- support
with individuals who live on their own, are living with family members, or are
in foster care. Our services are provided throughout the Greater Twin Cities
metro area in the individual’s home and in the community. We focus on the development of personal
outcomes plans with a commitment to realizing potential.
